STUDENT WAIVER OF LIABILITY *

I, the undersigned, acknowledge and recognize the inherent risks of participating in the Programs and Activities of Guttenberg Arts (GA) in a professionally equipped art studio, and assume all responsibility and risks inherent to this participation. I, for myself, my heirs, and legal representatives, knowingly and voluntarily agree to release all liability and agree not to sue, hold harmless, or hold responsible Guttenberg Arts and all Agents of GA, including but not limited to employees, owners, GA board members, and resident artists, from all claims, responsibility and liability for all damages and injury (including paralysis and death) to persons (including myself) or property (personal and otherwise) resulting from the contact with or use of tools, materials, equipment, facility, or from any other cause whatsoever while participating in GA Activities and Programs. This includes claims of negligence, resulting in any physical or psychological injury (including paralysis and death), illness, damages, or economic or emotional loss I may suffer because of my participation in Activities or Programs, including travel to, from and during. Personal Property: GA will do all that is reasonably within its power to provide a safe and secure environment but cannot guarantee the security of personal property at all times. I understand that protection of my personal property from theft, misuse, misplacement, damage or destruction and any other circumstance that may cause deterioration, depreciation, or need for replacement is my sole responsibility. I agree to hold GA and all Agents harmless from any and all claims, including attorney’s fees or damage to my personal property that may occur as a result of my participation in this program, including travel to, from and during the program. If GA or its Agents incur any of these types of expenses, I agree to reimburse these parties. Medical history: In case of emergency or accident please state any allergies, current use of medications, pre-existing conditions, or mental health / medical information or history that is important for GA to know in order to provide for a safe environment for all participants and staff in GA activities and programs. If I need medical treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. I understand the legal consequences of signing this document, including (a) releasing GA and all Agents from all liability, (b) promising not to sue GA or any of its Agents, and (c) assuming all risks of participating in Activities and Programs, including travel to, from, and during Activities and Programs. I understand that this document is written to be as broad and inclusive as legally permitted by the State of New Jersey. I agree that if any portion is held invalid or unenforceable, I will continue to be bound by the remaining terms. I am entering into this agreement at my own risk and in full understanding of the inherent risks of a professionally equipped art studio. The Undersigned acknowledges that they have read, understand, and will abide by all rules and regulations set forth by GA and its management.

BEHAVIOR EXPECTATIONS *

At Guttenberg Arts we stress the importance of safety and respect, creating an environment where children feel comfortable and happy. When confrontation or behavior problems occur, educators will try to find a solution for a negative situation gently by talking with children. There are times when a child may be asked to spend some time out of the workspace to reflect on a situation. When these situations become more serious, parents may need to be contacted. In general, the following behaviors are expected of every child: Respecting the property of others, including program materials Respecting and following the directions of summer staff Respecting yourself and other children All children must be potty trained before attending Guttengarden Art Camp. Instances of physical aggression and/or concern for safety of any camper can lead to immediate termination of participation in camp. In addition, if parents are unwilling to honor camp policies or hinder the relationship between their child, his/her peers and/or teachers, the administration may determine that termination from the program is necessary.
